Enrollment Checklist The following documentation is required for enrollment in The Academy of Moore County. Copies of this information must remain in the student file.

- Immunization Records
- Proof of Physical Address
- Custody Documents (if applicable)

The following forms will be sent home and collected at the beginning of the school year:
- Technology User Agreement
- Impact Aid Form (Military Families)
- Household Income Survey
